Service Description

Our advanced practice provide at Pure is able to provide pre-operative evaluations and surgical clearance. This service includes a general physical exam, orders for required diagnostic testing, and communication of your results directly to your surgeon. Schedule your appointment 3-4 weeks prior to your surgery date. Surgical clearance is based on provider assessment and interpretation of diagnostic results. To refunds are provided for individuals who cannot be cleared for surgery. Often times, the surgeon may request results instead of an actually letter of clearance. In this case, he or she will determine if you are fit for surgery.

Cancellation Policy

Appointments may be canceled up to 24 hours before the scheduled appointment time. Full payment or a deposit is required to book. All deposits are NON-REFUNDABLE. Reservations/Appointments: Advanced booking prior to your arrival is recommended to secure your preferred date and time of treatment. Based on availability, walk-ins are welcome. Please arrive 10 minutes prior to your scheduled start time. Late arrivals will only receive the remainder of their scheduled session. Cancellation Policy: Kindly provide us with a 24-hour cancellation notice for your appointment. Rescheduling of appointments is subject to availability. Cancellations or rescheduling require 24-hour notice for any singular appointment or service. Canceling multiple services, packages, or Parties of 2 or more requires 48-hour notice. Any cancellation with less than the required time may incur a 50% cancellation fee. No part of the initial deposit will be refunded for cancellations within 24 hours prior to the scheduled appointment. A full fee will be imposed for a “no-show”. Your time is just as valuable to us as our own, and we understand there are always emergency situations that have to be taken into account, which is why we appreciate your communication with us.
